Option 2: Going Through Your Insurance
We're here to help you maximize your insurance benefits for DME!
To ensure we can process your order through your insurance, your healthcare provider will need to send us some essential documentation.
Provider Submission Required: Please ask your healthcare provider to send the following documentation to Valor Medical Solutions via fax at (702) 921-0111:
A Detailed Order for the DME: This is more than just a prescription; it's a comprehensive order specifying the exact DME item needed.
Demographic Information: Your provider should include your patient demographic details.
Your Last Clinical Note: This provides necessary medical justification for the DME.
Valor's Review & Processing: Once we receive all the required documentation from your provider, our team will review it and work directly with your insurance company to get as much coverage as possible.
Important Note: Even if you have a prescription, we still require the detailed order, demographic information, and last clinical note directly from your doctor's office to process through insurance.
